The foundation of periodontal care and the starting point for all patients presenting with gum concerns. Removal of all supragingival (above gum line) tartar, calculus, plaque, and surface staining using ultrasonic and hand scalers, followed by tooth polishing to smooth the enamel surface and resist future plaque accumulation. Recommended every 6 months as part of a periodontal maintenance programme.

Scaling and root planing is the primary non-surgical treatment for mild to moderate periodontitis. It is carried out under local anaesthesia for complete comfort, using ultrasonic and hand instruments to remove subgingival (below gum line) calculus and biofilm. Root surfaces are planed to a smooth finish to prevent bacterial reattachment. Typically completed in two to four sessions, treating one quadrant or half of the mouth at a time. Reduces pocket depths, controls inflammation, and halts disease progression in the majority of cases. Post-treatment review at 6 to 8 weeks assesses healing and residual pocket depths.

In cases of aggressive or refractory periodontitis, localised or systemic antibiotic therapy may be prescribed alongside mechanical treatment to eliminate bacteria that cannot be reached by instruments alone. Locally delivered antibiotics are placed directly into periodontal pockets at treatment; systemic antibiotics are prescribed when generalised aggressive disease is present. Antibiotic selection is based on clinical presentation and, where indicated, microbiological testing.

Where non-surgical therapy does not fully resolve disease activity — or where pocket depths and bone loss are too severe to manage conservatively — periodontal surgery is required. Our surgical team in Antalya is experienced in the full range of periodontal surgical procedures.

Flap surgery (open debridement) involves gently reflecting the gum tissue to allow direct access to the root surfaces and bone, where deep calculus deposits and diseased tissue are removed under direct vision. The gum is then repositioned and sutured, significantly reducing pocket depths.

Bone grafting is performed where bone loss has occurred around a tooth. Grafting material is placed to encourage regeneration, with guided bone regeneration (GBR) membranes supporting the healing process. Frequently combined with flap surgery in the same procedure.

Gum grafting addresses gum recession that has exposed root surfaces. Connective tissue grafts harvested from the palate or sourced from a tissue bank cover exposed roots, reduce sensitivity, and improve the appearance of the gum line. Also used to increase the width of keratinised gum tissue around implants.

Laser therapy is an adjunctive treatment used alongside conventional periodontal procedures to decontaminate periodontal pockets, stimulate tissue healing, and reduce bacterial load with minimal discomfort. A diode laser targets and destroys bacterial biofilm within the periodontal pocket, promotes regeneration of the junctional epithelium and connective tissue attachment, and reduces post-treatment discomfort and swelling. Used as an adjunct to scaling and root planing or flap surgery, not as a standalone replacement.

Following active periodontal treatment, long-term supportive maintenance is essential to prevent disease recurrence.
